Claim on Consignment by Road:
- Claim Intimation / Registration.
- Copy of policy.
- Claim form duly filled up and signed.
- Copy of invoice/ packing list/ consignment note.
- List of items damaged along with bill.
- Police report if vehicle is involved in an accident.
- Damage certificate of transporter/ short certificate.
- Notice to the transporter with acknowledgement.
- Subrogation letter.
- Surveyor report with photographs.
- Inspection report.
Claim on Non-Delivery Case:
- Non-Delivery cases original consignment note.
- Non-Delivery Certificate from the road transporter/carrier.
- Certificate copy of the remarks in the delivery challan.
- Notice of claim lodged to the carrier.
- Subsequent correspondence with the carriers.
- Appointment of field officers and surveyors.
- Role of field officers and surveyors.
Surveyor log book/ salvage collection and disposal.
