Document Checklist - Marine Insurance Claim

Documents required for Marine claim

Claim on Consignment by Road:

  1. Claim Intimation / Registration.
  2. Copy of policy.
  3. Claim form duly filled up and signed.
  4. Copy of invoice/ packing list/ consignment note.
  5. List of items damaged along with bill.
  6. Police report if vehicle is involved in an accident.
  7. Damage certificate of transporter/ short certificate.
  8. Notice to the transporter with acknowledgement.
  9. Subrogation letter.
  10. Surveyor report with photographs.
  11. Inspection report.

Claim on Non-Delivery Case:

  1. Non-Delivery cases original consignment note.
  2. Non-Delivery Certificate from the road transporter/carrier.
  3. Certificate copy of the remarks in the delivery challan.
  4. Notice of claim lodged to the carrier.
  5. Subsequent correspondence with the carriers.
  6. Appointment of field officers and surveyors.
  7. Role of field officers and surveyors.

Surveyor log book/ salvage collection and disposal.
